This survey is being sent to you or your child as you will have an Education Health & Care Plan (EHC Plan), you will have therefore either been involved in the EHC Needs Assessment Process or the Conversion of a statement into an EHC Plan.   We would be grateful for your views so that we can continue to improve and develop the EHC process.

The Children and Families Act 2014 made changes to the way Local Authorities and Health providers support children and young people with Special Educational Needs and or Disabilities (SEND).

From September 2014 changes introduced included replacing Statements and Learning Difficulty Assessments (LDA's) with EHC Plans. 

An EHC Plan is a forward looking statutory document that captures the long term aspirations/hopes of the child/young person (0-25 years), as well as the necessary provision to meet their SEND needs.

EHC Plans should show how services will be delivered as part of a whole package and explain how best to achieve the outcomes sought across education, health and social care for the child / young person.

A strong emphasis is placed on the involvement of children/young people and their families.

Everyone who completes the survey and leaves their email contact details will be automatically entered into a draw and 3 lucky winners will be picked at random to receive a family ticket for the Hexagon Theatre.
